This is a complete list of all client and Dashboard-related request forms, as well as other internal forms that FareHarbor employees may need to access. You may also want to check out the Dashboard Changes & Responsibilities page to learn more about which teams handle which requests.

Client-facing
Below is a list of request forms that can be shared with live clients, and which team manages each type of request.
To request a change to one of these forms, first get approval from leadership of the managing team, then submit a request to the Help Page Request Form.
For client-facing forms that are translated, any requested copy changes (such as new fields or edits to existing field text) will need to be approved and translated as part of this process. This may increase turnaround time by 1-2 weeks.

Membership Build Request (Support Team)
- Note that each form submission is meant to capture one membership type. If a client has multiple memberships, they will need to submit the form multiple times.
FH Dock Request (Support Team)
The following commonly used forms are hard-coded into the FareHarbor content website. Updates to these two forms require code changes and translation. Keep in mind that these forms also affect many teams and systems (Zendesk triggers, automated team ticket assignment, etc). To request a change to one of these forms, please submit a request to the FH.me Idea Box.

Sales/Leads
Support Sales/Demo Request Form: Non-Live Prospects
- For Support agents submitting a demo-request from non-live companies who are not existing leads in Close.

Dashboard/Website Requests

- Requests require Sales Manager approval and are reviewed by the Build Team Manager and Director of Onboarding. More info here.
New Dashboard Request for LIVE clients
- This submission form goes to Sales and Onboarding. From there, the request is funneled to the AE or an OB based on predetermined criteria. More info here.

Strategic Partnerships
Strategic Partnerships Inquiry Form
- Use this internal form to suggest to the Strategic Partnerships Team to reach out and form a partnership with a partner/affiliate that distributes tours/activities/attractions to end customers. The distribution partnership types that are handled by the Strategic Partners are new External API connections and FHDN partners. Please make sure to:
- Check if the partner is not live on the FareHarbor API here. If they are not listed in the help page then fill out the form above. The SP team uses these submissions to prioritize which partners to reach out to.
- Fill out the form individually for each company you are suggesting.
- Use this internal form to suggest to the Strategic Partnerships Team to reach out and form a partnership with a partner/affiliate that distributes tours/activities/attractions to end customers. Software Partnerships Requests
A lead/client may ask us to integrate with another third-party software that they use, like a staff scheduling software or a property management system. Use the form below for all non-distribution partners integration requests:

- Updates completed on client’s live Dashboard that need to be updated on their live FareHarbor Site
