This form is used to add new leads with the lead status "Partner/Affiliate'' to Close. If the company is already in Close, do not submit this form. You can check all submissions in this view: https://airtable.com/appqdgC4KC0n9GWcb/shrvNgYRR1GW4V5Qo/tblq7IaZJrboBY3Mu

Partner/Affiliate leads are submitted mainly by the Channel Support, Strategic Partnerships, and Connectivity teams. Submissions are handled by the Lead Generation team. If you are submitting a new partner/affiliate lead that has a FareHarbor Dashboard, select the Affiliate Setup (s) that best apply in their case (i.e. Affiliate Dashboard, or ASN - Live, or API Partner, etc.

Make sure to include the contact details of your point of contact at the affiliate company (i.e. name, position, email address, and/or phone number). This can be taken from a Zendesk ticket or their email signature. For any additional contacts you have, make sure to include those in the notes at the end of this form.
